ANONYMOUS CHILD SAFE REPORTING FORM

This form is for people in the school community to report a concern regarding the safety and wellbeing of a child.  While we encourage our community to provide names, we acknowledge that in certain situations you may prefer to report a concern anonymously.

The information in this form will be used to determine if a student is at significant risk of harm. Allegations will be responded to on face value and so it is important that the details included are accurate. Please note that in certain circumstances this information may result in a report being made to the Department of Communities and Justice (DCJ) and/or the Police.

Areas of concern include:

  • Physical Abuse

  • Sexual Abuse

  • Neglect - suspect child’s needs are not being met

  • Psychological Harm - experiencing emotional or psychological stress and is a danger to self or others

  • Danger to Self or Others - including self-harm or suicidal ideation

  • Relinquishing Care - parents refusing to care for their child

  • Carer Concern - substance abuse, domestic violence

  • Unborn Child - welfare of an unborn child

This form will be sent directly to the Principal.  Please note that if the concern involves the Principal then contact the Chair of the Carinya Board directly.
